Had CHF, Kidney Transplant, Intubated Due To Pneumonia, Cdiff. Bleeding From Mouth, Rectum Occurred. Why?

Hi
My uncle recently passed away
He was 61 years , had CHF, kidney transplant. He was intubated for 3 weeks during which he had penumonia and cdiff. Antibiotics were being given. He had a traechstomy done and 2 days later he started bleeding from the mouth for 3 days. They gave him blood, FFP , cryo etc and on 3rd day he was ok.Then after 2 days he had rectal bleeding and doctors gave up hope and he passed away. Why could the bleeding from the mouth and rectum would have occuerd.? His heart and kidney function wass stable all along. Lungs were congested but he was on lassix.

OB & GYN Specialist 's  Response
Hi,
Thanks for writing to us.
This bleeding may be because of deficiency of clotting factor because in this condition his liver is congested and his liver function is deranged.
In this condition he should continue on blood transfusion, FFP and cryo etc.
Good luck.
Take care.

Pain Medicine & Palliative Care Specialist Dr. Kerry Pottinger's  Response
Hi,
I'm sorry that your uncle passed away. From the description you have given, I think he probably developed disseminated intravascular coagulation. In this condition, there is uncontrolled bleeding despite blood, FFP, clotting factors and cryoprepcipitate. It often occurs with multisystem failure which is what your uncle appears to have had. It is often an indication that nothing further can be done.

Dr K A Pottinger,
