Hello and welcome to HCM,
The symptoms of palpitations, dyspnea, left sided
chest pain suggest some cardiac problem.
An ECG needs to be done to know any abnormality of the heart.
If no abnormality is detected on the ECG, holter studies and / or
echocardiography need to be done.
Left
shoulder pain and sharp pain on the left chest suggest anginal pain.
Associated pain in the calf can be due to
deep vein thrombosis.
Doppler studies are required to know the exact cause and site of the thrombus (if any).
A blood glucose levels need to be done as diabetes may co-exist with cardiovascular diseases.
Your history is highly suggestive of disease process of the cardiovascular system.
you need to
consult a cardiologist for further work up.
However, do not delay as the angina that you are suffering may be unstable.
It may precipitate without any warning.
