How Can A Painful Lump Near The Anus Be Treated?

I have a lump next to my anal it hurts real bad and it arrived during a bout of constipation... I get constipated a lot and have had one internal hemeroid a few years ago...this is larger and hurts more....I am cleaning it with cleansing pads and using hemeroid all cream since Saturday....but I am nervous about it....I can t sit,,,,it is kind of large considering the area.
