How Can Chronic Insomnia While On Fentanyl Be Treated?

My 90 year old dad has Alzheimer s. He also has rheumotoid arthritis, scolosis of the spine. He as taken predisone and methotraxate for RA but ened up with rectum cancer in 2012. That was taken care of and he was taken off methotraxate and now just presinone 10mg and extra strenth tylenol and a 50mcg fentanyl patch. He is on hopsice for a variety of reasons. He is not verbal and just makes a lot of noise. He is restless and doesn t sleep well at night. Have given him lorazapam up to .50 to help with anxiety helps sometimes. Hospice wants to try a sleep aid or anti-convulsant like Depakota. I really dont want to add anymore drugs if I can help it. But if we would add a drug to help sleep, what might be recommended to not knock him out and iteract with the Fentanyl? YYYY@YYYY

Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

In geriatric age and if this is 90 years with multiple pathologies (Illness) psychological disturbances, multidrug therapy is not advisable, now for the sleep patient already taken Lorazepam now nothing more left looking to all the clinical history. You can try Diazepam if it could works.

Hope I have answered your query. Let me know if I can assist you further.
Regards,
Dr. Akhtar Husain
