How Can Day Time Sleepiness Be Managed?

I m a 64 y/o female on suboxone 8/2mg daily, gabapentin 600mg 3/day, trazadone 200mg/pm with tribenzor 40/10/25 about every third day (i monitor my bp often thru the day and don t take it when it s lower than 110/70, and it does dip to 80s/60s some mornings but i recheck it often and take the tribenzor later if needed) I have hypertrophic anterior horns cervical spine impinging on my chord diagnosed 3 years ago but asymptomatic other than neck pain and headaches. I was diagnosed with fibromyalgia and took MSContin. Dilaudid and other strong opiates until I reached a point when they d be gone in 4-5days and I just couldn t go on like that so i sought addiction treatment and now take suboxone over 3 years now and doing great. It s been creeping up on me but this fatigue I feel scares me as I actually am now falling asleep a second or two while at red lights or while trying to read or sitting. I also see vibrant hallucinations when my eyes are closed. Now having trouble staying on task, my mind wanders top another task quickly do i forget what i set out to do making me fear this will interfere with my nursing critical thinking and performance. My doc prescribed prescribed adderall 15mg 2/day to help. It helps with the daytime sleepiness but seems to slow my thinking. Also I feel even more severe fatigue about one hour after taking. What else is there that could help me stay more awake and have clearer thoughts and memory without altering my personality?

Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

I've read carefully query and I'm pretty sure your daytime sleepiness is caused by the drugs you're taking Gabapentin-Trazadone-Suboxone. So the plan is to lower the dose or stop taking any of them, but first you should consult with your primary doctor for a better management of fibromyalgia and opioid addiction, so the the use of Adderal will be needless. Afterall, you will have a better day.

Hope I have answered your query. Let me know if I can assist you further.
