How Can Dementia And Alzheimer S Disease Be Managed?

I am a caregiver for an 85 yr old woman who has Lewy Body Dementia/Alzheimers. I would like to prepare a daily report for the family so they are aware of her: behavior, moods, thinking skills, movement. Is there a template that I can follow or use as a guide?
