How Can Pain In The Buttocks Be Treated?

I have piriforis muscle pain and have taken therapy 3 or 4 times n the past 3 or 4 years. Have taken 2 shots in my back as I also have spinal stenosis and was treated for that. Therapy has helped but the pain alway comes back and never goes completely away. Have been to orthopedist. Is there ANYTHING that can be done. I also have osteoporosis pretty bad.
