I am a 54 y.o. female with panhypopituitarianism and a damamged hypothalamus, etiology: optic chiasmal glioma cystic astrocytoma (probably born with it, it was diagnosed just prior to my 9th birthday) 2 craniotomies (aged 9 y.o. and 23 y.o. and 5000 rads cobalt therapy administered bitemporally over ten weeks at age nine. I now have truncal obesity despite gastric bypass surgery, but my arms and thighs are thin, with muscle wasting. I have been diagnosed with a growth hormone deficiency. I also have type II diabetes and am hypothyroid. Do you think the truncal obesity is mostly internal or external to the abdominal cavity, and what should I do to reduce the fat in this area? I believe that the fat reduction would help me to better ontrol of my diabetes.

Hello . Thanks for writing to HCM.
I read and understand your question.
I try my level best to solve it.
As you are suffering from panhypopituitarism , it is one of the symptom is truncal obesity. So if you want to reduce your truncal obesity than first of all get treatment for this cause. You should take endocrine replacement therapy for it.
Endocrine replacement therapy should aim to mimic the normal hormonal milieu as far as possible, thus improving symptoms while avoiding over-treatment. It can be need to modify treatment during intercurrent illness is vital.
I also suggest you to restrict oily , fatty food like butter , ghee etc , also avoid junk food. Also do exercise regularly like walking and running.

I hope my advice will help you a lot.
Thanks and Regards
Dr Vijay
