How To Cure Sentinel Pile?

hi i am a 30 year old female i developed an anal fissure after my delivery , initially it used to bleed but now that it has been 6 months the bleeding is gone but a sentinel pile has developed , i am having lactulose syrup 10ml in combination to softovac 2.5gm daily and am now not facing any pain while passing motion but i can still feel a lump in my anal area , i wanted to know if at all sentinel pile resolves on its own or will i have to go for a surgery only
