Your brother symptoms are really complex and could be related to different possible disorders: Dementia, a stroke or any other neurodegenerative disorders. I would like to know if his symptoms have began gradually or in an acute way? Coming to this point, I would recommend consulting with a neurologist for a physical exam, some cognitive tests and a brain MRI. Further blood lab tests (complete blood count, vitamin E and vitamin B12 plasma levels, blood electrolytes, thyroid hormone levels, etc..) may be needed.
Hope you will find this answer helpful! I would be glad to review your tests for a more professional opinion! Best regards!
