How To Manage Difficulty In Speech, Stumbling / Stuttering, When Having A Conversation?

Hi, whenever I attempt to speak out loud to an individual or a group of people I stumble over words, and can't think of what I want to say. On the other hand when I just think to myself or go over a conversation in my head I have no problem. I can have an amazing conversation in my head but when I try to regurgitate it, it goes down in a fireball. Any help would be much appreciated.Thanks.
