First of all there can not be piles that continue to pain for such a long time of 2 years hence you need a proper clinical evaluation and examination, per-rectal digital examination, ano-proctoscopy and
colonoscopy as you are 36 years and suffering for such a long time.
I would also advise you the following:
Sitz bath in a tub with warm water.
Laxatives to keep the bowel soft and loose.
Anti-inflammatory with painkiller properties.
Tests of blood,stool and urine.
Surgery of the prolapsed piles if present.
Anal dilatation if there is
anal fissure too.
Further management as per the reports and clinical findings.
