General Surgeon 's  Response
Hello,

As per my clinical experience, the extensive skin lesion from the vagina to anus with urinary tract infection indicates underlying genital infection mostly sexually transmitted or other, systemic issues as diabetes or else. In our clinic, we immediately take basic lab tests, swab test of the lesion and manage accordingly.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel
