You can safely add tab zytanix which contains metalazone 5 mgs. Considering the history & symptoms narreted by you it suugests fluid overload due to the left ventricular failure and the decrased urine output may cause more fluid retention and may worsen the condition. As he is already on torsemide 100 mg Addition of the tab zytanix is wise decision.
One more tab you can add is tab nephrosave Twice daily ( considering derranged creatinin which may benifit kidnies )
& if it dosent iprove the urine output then it is advisible to hospitalise the patient on priority basis.
