Is Leaking Urine In Small Amounts Normal?

Hi, I am a 27 yr old mom of three. I have noticed over the years that my urine bleaches my underwear, I assume that it is urine as it leaks out in small amounts. Is this normal? What can I do to prevent this it is so uncomfortble to have wet panties during the day and night. Should I be concerned?
