Is Skin Tag On Lower Back And Lump Sign Of Infection In Kids?

I have a 2 year old who has a skin tag on her lower back. It has a lump underneath it and the skin is red and hot. I have taken her to the doctor and they prescribed antibiotics (4 days ago) but it doesn t appear to be helping. We can t get into the dermatologist for another 2 weeks. What should our next action be? Could this be more than an infection?
