On Simvastin, For Severe Night Tremors. Got Severe Pain In Right Calf Muscle, Can Hardly Walk. Causes ?

Hi Dr. , My husband is on 20mg of simvastin. For the last year or so he has had night tremors that are quite severe. Recently, in the last couple of months he has developed a severe pain in his right calf muscle. He went to urgent care thinking it was a clot and was sent to the hospital for a test but nothing was found. Still, the pain persist off and on. On his last visit to his regular Dr. he was told to drink more water. Water is all he drinks, so I don't think this is the answer. Sometimes, the pain is so severe he can hardly walk. And it's worse when he get up in the morning. Can you give us any insight as to what might be causing both of these conditions.

Hi, thanks for using healthcare magic

Statins including simvastatin , have the potential to cause muscle aches, cramps or weakness.

This can sometimes cause an increase in creatine kinase blood levels but some persons can have this muscle pain and the levels remain normal.
Creatine kinase is an enzyme found in the brain, heart or muscles.
It levels can go up with damage to these areas.

If the pain and tremors are related to the simvastatin, they will stop within a few days of discontinuation of the drug.

You can consider speaking to your husband's doctor to find out if this is an option to rule out the medication as a cause.

I hope this helps

Orthopaedic Surgeon, Joint Replacement Dr. K. Naga Ravi Prasad's  Response
Hi, thanks for writing to HCM.

From the description of symptoms, I feel that SIMVASTATIN is the cause for the your husbands severe muscle pains.

Simvastatin ocassionally causes MYOPATHY which presents as muscle pain, tenderness or weakness. Very rarely this myopathy may take the form of rhabdomyolysis and may end in acute renal failure.

CPK (Creatine phospho kinase) enzyme level estimation in the blood helps in diagnosing the muscle damage caused by STATINS. A raised value indicate muscle damage.

So I suggest you to consult your treating doctor and discuss the issues.

Hope this information is helpful. Good luck
