Pain When Pulling Out Tampon. What Did I Do?

I always use the super absorbency tampons no matter what my period flow is because it is different from one hour to the next. yesterday and today, I have experienced extreme pain while pulling out my tampon. The tampon is dry, but everytime i go to the bathroom to urinate, my urine gets on my tampon and then i HAVE to take it out, even if in theory it is not READY to come out. so when i pull on it, of course it s dry and hard to pull out. and as i pull on it, It feels like it is taking my cervix with it or pulling my tissue from waaaayyyy up where my cervix is. its very painful....and tonight when i took my last tampon out and tried to put a new one in, it hurt just as much putting it in as pulling one out! what did i do? i obviously tore something, or irritated something...
