Hello.I think that tight perianal muscles,Your doctor refers to anal sphincter.
To diagnose of anal sphincter disfunction he must request diagnostic test for measure anal pressure(anal manometry). He must request too diagnostic test to see intestinal tract(intestinal endoscopy). After those test, will be more reliable diagnose. Now you must eat diet rich in water, vegetables and fruit(not apple and banana);take some laxative drug(for example plantago ovata)and go to your doctor for examine and request diagnostic test.
I wish you good health.
