Diabetologist 's  Response
Dear sir

As per your query your current medicines you are taking
1. metformin 500 mg three times a day

Fasting sugars are high, it might be due to two reasons
1. Increased hepatic glucose output.
2. decreased insulin secretion.

Since metformin is a insulin sensitizers, it wont improve the insulin secretion .

To decrease fasting sugars, add bed time basal insulin or add a secretogogue drugs like glimeperide or glibenclamide at night times.

Thanks.
