Recurrent Urinary Infection. Done Ultrasound. Prescribed Trimethroprim. Help

Hi. I m a 27 year old female, 5 4 weighing 9st 2. I ve been having recurrent urinary infections for about 9 months now. My doctor has given me a lot of antibiotics and referred me for blood tests for glucose , thyroid , liver & kidney and FBC. I have also been for an ultrasound scan on my bladder , urethra and kidneys. All have come back clear. Most recently my doctor has put my on a 3 month course of trimethroprim. I no longer have blood or pain when I pee but I am still suffering from a frequent urge to pee, and pain when I get the urge in my abdomen. I also suffer from IBS which usually leaves me constipated. I have noticed that when I am constipated this pain and frequency in my bladder is much worse and that after I move my bowels and finish up in the bathroom, I feel like my bladder is full again straight away. My perineum is also really swollen and has bled bright red blood a few times. It hurts to have sex. I have one sexual partner and don t have an STI. Thanks for your help.

Urologist 's  Response
Hello
Thanks for your query.Based on the facts that you have posted it appears that you have Chronic Infection of the Urinary Bladder and have been prescribed Trimethoprim for three months.
You have done almost all the necessary test which turned out to be negative.

Please get your routine urine test and urine culture done to find out the organisms causing this infection and antibiotics to which they are sensitive to.

In the mean while start taking broad spectrum antibiotics like Cefexine ,urinary antispasmodics like Tolturadine and urinary antiseptics like Nitrofurantoin along with urine alkaliser and switch on to appropriate antibiotics as per culture report.

Ensure to drink more water. To keep your urine dilute This will help to control dysuria.

Please consult qualified Urologist for clinical evaluation ,he may do endoscopic examination of the bladder and take biopsy of the bladder to rule out Interstitial Cystitis.
