27 yo female, no history of disease, smoker and occasional amphetamine user. having severe lower back pain similar to labor cramps but not pregnant. also moderate pain in neck, arms (wrists/hands) and shoulders. pain is steady and constant. slight lightheadedness. went to ER yesterday concerned about heart attack but sent home, normal oxygen, blood pressure, heart rate, ekg, blood tests.
It is very important from clinical scenario to exlude Polymyalgia Rheumatica. It actually presents as such with sensation of fever, lower girdle pain and shoulder pain and tightness. Doing an Erythrocyte Sedimentation Test (ESR) shall be important to further strengthen the possibility for this diagnosis. Other rheumatologic conditions like rheumatoid arthritis and lupus, sarcoidosis and mixed connective tissue disease are all possibilities.
