Orthopaedic Surgeon, Joint Replacement 's  Response
Hi, thanks for writing to HCM.

Spontaneous onset of pain along with redness and warmth indicates an infective or inflammatory proces in the involved area.

Few blood investigations along with either Ultrasound examination or MRI scan of the foot will diagnose your condition.

Consult an orthopedic surgeon for further workup and management.

Hope this information is helpful. Good day

Orthopaedic Surgeon Dr. Vaibhav Gandhi's  Response
Hello

Thanks for writing to Healthcare magic,

I have studied your case,
I would like to keep possibility of gout first as per your history and symptoms.
Yes another possibility can be infection which is not resolving by penicillin ,you my need to change antibiotic in that case.
How is your toe any swelling there?
I advise to go for blood inv like complete blood count ,serum uric acid level.

Hope I have answered your query. Let me know if I can assist you further.

Take care
