Should I Be Concerned About The Vomiting And Blood In Stool Of Baby?

My son is 7.5 months old. He is mostly breastfed, but he does get formula for nap and bed time. He also eats some baby food. The past few days he has thrown up and spit up after nursing. Today I found a mucus blood clot in his diaper. I want to make sure he is okay

What about his general condition? Is he feeling restless or lethargic? Is there ant decrease in oral acceptance? Or any other symptoms like fever?
These features may indicate a systemic insult like infection which need evaluation.
