You have several reasons to be concerned, not so much from the weight as from the other problems you mention. The heavy smoking and the wheezing of his lungs suggests he may have a problem such as COPD (chronic obstructive pulmonary disease, such as emphysema). If most of his weight is carried in his belly, this also increases his risk for what's called metabolic syndrome, which is like a prediabetic condition. With the blurry vision, I would also be wondering if he actually has diabetes. I think it would be a good idea if he got a complete physical exam by his doctor and had bloodwork checking these conditions, including his cholesterol. He is at an age where heart and diabetes problems start appearing, and where he should start getting regular screening exams such as colonoscopy for polyps and colon cancer. Encourage him to get a thorough checkup if you can. Hope this helps.
