Should I Use Clindamycin Phosphate And Benzoyl Peroxide For Intertrigo?

hello i am suffering from what my primary care doctor says is intertrigo under my breasts and my underarms, she first prescribed miconazole nitrate 2% cream and hydrocortizone 2.5% cream which didnt help much,then she prescribed nystatin 100000 usp cream and referred me to a dermatologist. He prescribed clindamycin phosphate 1% topical lotion and benzoyl peroxide 5% gel...which are both usually prescribed for acne..my question is would this be something that is usually prescribed for intertrigo or should i seek a second opinion ?...thank you in advance for your time :)

thank you for asking your concern madam and welcome to hcm. intertrigo is inflammation of the skin folds, axilla, space between fingers and it causes due to sweating, irritation and repeated friction of the the skin. moisture is the main thing which enables infection. among these infection fungal infection is the most common followed by bacterial and viral. you already used antifungal but problem is you also used hydrocortisone cream and this is the main contradiction. sometime, miconazole attacks on fungus but hydrocortisone helps to grow fungus so even one drug is trying to kill other may prevent it. then it is best to use single drug only.
i would suggest, you can use single antifungal cream like miconazole or clotrimazole without any combination. use it three times a day. at the same time keeping area dry is most important so use a hanky when you feel moist. you can take antibiotic tablet along with it if it is secondarily affected by bacteria, like capsule amoxicillin 500 mg three times a day for 5-7 days.
skin scrap for fungal hyphae under KOH mount is the test which you can do if possible and at the same time do test for diabetes, if not possible this time it is ok. you can withhold taking clindamycin cream for now because if fungal infection is there it is not going to work either.
