Some individuals have short frenulums (skin that attaches the foreskin to the base of the glans penis). In such situations the frenulum prevents the full retraction of foreskin during intercourse resulting in a tear. This requires treatment by a doctor (surgical) and has two options for treatment.
1. Circumcision
2. Frenulum incision (thus releasing a bit of the tension from the foreskin)
In my opinion a circumcision would be the best option, for both your sexual life as well as personal hygiene.
