Small Child, Fever, High Body Heat On Head And Torso, Cold Arms And Legs. Heart Condition?

My 2 year old son who is also a heart patient has been running a fever between 100-101 the past 2 days this morning when I checked him he had a fever and the only thing feverish was his head and torso. His hands arms legs and feet were cool to cold what does this mean should I be calling his cardiologist or is this normal.
