Sudden Weight Loss, Excessive Thirst, Uncontrolled Urination. Diabetes?

my guy friend is 56 yrs old. in the last three months he has lost 50 lbs. or 3 or 4 pant sizes. he is constantly drinking fluids like no tomorrow. when i did his laundry i noticed he's been wetting himself a lot and his pee soiled under clothes show a hard, stained white crusty residue. not a little spot but a large area which i feel is a concern. what is your thought on this. my thought is that he has diabetes and a problem with his prostate. if this is true how serious is this.

Hello,

Your friend is a Diabetic and he is losing weight rapidly.
He has increased thirst and in urinates very frequently.
All this symptoms may be due to uncontrolled blood sugars or he might have developed diabetes related complication like Diabetic nephropathy.
Test his renal function.
Ultrasound KUB may help in ruling out prostate related problems.

Hello,
Thanks for posting the question.
At the age of 56 yrs prostate is not uncommon but excessive thirst is not there.
Incontinence of urine is common in
Benign Prostate Hypertrophy ( enlargement of prostate but in this case urgency if urination is there but patient pass very small amount of urine( volume is very less) , hence there is no chances of dehydration.
Diabetes , at the age of 56 yrs this is very common, if patient is obese than more chances of diabetes.
If the life of patient is very tense from long time chances of diabetes are more.
Query is that crusty residue is there so it may be due to
Diabetes,
Phosphaturia ( passing of phosphate in urine in excess amount.
Proteinuria
Pus cells in urine.
Please get in physical examination by a surgeon to rule out prostate enlargement.
Blood examination to rule out diabetes, any infectious diseases.
Good luck. Dr. HET

Hello,
Although there can ba various causes for the weight loss in a person but if somebody complains of weightloss with additional complaint of increased thirst and increased urination, then diabetes seems to be the most probable cause behind it.
You must consult a doctor and get your fasting and PP blood sugar levels estimated.
Along with this get lipid profile, kidney function tests and urinalysis also done.
Uncontrolled diabetes results in various types of future micro as well as Macrovascular complications which causes damage ton various organs like kidneys, eyes, heart, blood vessels, nerves etc.
Once you are found to be diabetic then take proper anti diabetic medications along with proper dietary restrictions and regular aerobic exercises for 30 to 45 minutes a day.
Thanks

Internal Medicine Specialist Dr. Akhilesh Dubey's  Response
Hi,
Welcome to HCM,
He is 56 year old and having excessive thirst with weight loss which is significant especially as it is unintentional.
Certainly Diabetes is first possibility but looking in to his symptoms and weight loss he should go for complete medical check up.
Through physical examination and history including internal examination and investigations like Complete blood counts,Fasting blood sugar and postprandial blood sugar, Creatinine ,Liver function tests, Urine routine analysis,X ray chest, abdominal sonography.
His staining of underclothes could be due to glucose if he is diabetic and even many crystals can cause this or even infection like UTI.
Further workup depends on result of his tests as this much weight loss may point to presence of malignancy in body?
Please take care
Good Luck.

Check fasting nlood sugar and postprandial blood sugar to rule out diabetes. Take ultrasound abdomen to rule out BPH. Do a Hiv test also .take an xray chest to rule out PT.consult a diabetologist immediately if fasting blood sugar is more tham 100. Now Diabetes is very very easy to controll. Somany good drugs are available.
