Suggest Better Treatment For Sentinel Pile

,i am 68yrs suffered from fissure in anus,whichdwas cured 01yrs back ,ihave developed sentinel pile which become painful on passing hard stool only usually.i am using 2percent xylocaine jelly,before passing stool and laxative preceding night, this does help. please advice.
