As per your explanation I realise she has got multiple issues. I suggest to have check on her blood pressure and sugar regularly first, and then do track on her kidney issue as well as uric acid level in her body. If it's on higher side you have only option of doing dialysis. This will be better guided by your doctor with all her reports. For paralyse attack and functional disability you can take help of physics therapist to make her physical independent for her daily activity.
But all this will take time so have patience. And take care

Here I see your wife is suffering with mile problem. And with out checking on current report difficult to suggest but I will try my best and you can also check with your treating doctor.
For blood pressure she has to be on BP controlling drugs, for sugar she has to be on medication or on insulin. Insulin is better option for patients who have multiple issues, for stroke still if she has functional disability then better to take physio treatment and with exercise like stretching, strengthening balancing coordination with special techniques like proprietary neuromuscular facilitation and motor relearning program she will improve further.
For kidney and renal problem as per the uric acid level in body and kidney functional ability. You need to take call on dialysis also.
One more holistic guideline I can give is make her do regular exercise and Pranayama kind of things with natural living means living in natural environment with good diet habits this will also help her a lot.
Take care
