Orthopaedic Surgeon 's  Response
Hello,
Welcome, and thanks for sharing your concern
I went through your query, and I feel, the chronic pain of your ankle will demand a battery of tests to come to the conclusion about cause
1 X Ray
2 Blood tests - CBC, ESR, CRP, RF.
3 MRI scan if any of above is abnormal.

its only when we go through in this systemic manner that we can find out the cause and give appropriate treatment

I hope my advice would have been useful, in decision making regarding your treatment, still if you have any clarifications or doubts feel free to contact back.
Thanks.
