Suggest Treatment For Graves Disease

Dear Dr. Topiwala, I was diagnosed with Graves disease in March 2010. My doctor put me on 100mg of ptu twice a day. I did a blood test 6 weeks later and the free t3 and t4 were normal however the tsh was 0.01. My doctor told me to remain on the same ptu dosage of 100mg twice a day. I then moved to another city due to my husband changing jobs. I had to wait for two months before I did another blood test, because I had to wait for my new insurance.So my latest results were: TSH 2.140, Free T4 0.79, Free T3 2.1 . My next appointment with my endocrinologist is not before 3 weeks, and I would like to know how much ptu I should take. I m sure that I should decrease the dosage as I m feeling a bit sleepy, tired, and lethargic all the time. Please, I would appreciate your feedback as I m so confused and can t wait for three more weeks until I see my endocrinologist. Thank you so much. Celine

Hello,

Considering your latest results and your actual symptoms (feeling letarig, tired and sleepy), I can say that you have developed iatrogenic hypothyroidism.

The follow up with your treating endocrinologist is very important after you start the treatment with PTU because it is necessary to decrease gradually the doses every 4-6 weeks, accordingly to the thyroid test results.

In your circumstances, I suggest taking only 50 mg PTU/day untill you see your treating endocrinologist.
