Suggest Treatment For IBS

I have IBS. Today and yesterday, I was constipated and took some Mylanta twice. Finally I had a bowel movement this evening. Heavy diarreah and lots of gas. I've had this before. Went and went, and finally took an Immodium to make it stop. Went a few more times, and the last time I noticed a black spot in the toilet and a bit of blood around it. I never noticed blood before even with the worst diarreah and constipation. This was followed by heartburn, which I rarely have. I took a Prilosec and a Bentyl a few minutes ago. It seems to have calm down some but I still feel the urge to go and crampy. I never have seen blood from IBS before though I've had some bad bouts with it for years.

Hi welcome to Healthcaremagic, From the symptoms I feel you must be having IBS-A or Irritable bowel syndrome with alternate diarrhoea and constipation. But before being diagnosed as Irritable Bowel Syndrome, you have to undergo investigations to rule out other gastrointestinal causes of your symptoms, if not done please reply we'll be able to guide you.
From your symptoms of alternate diarrhoea and constipation I feel the treatment will need a change in diet followed by drugs.
In diet, completely avoid milk and milk products since most patients with Irritable bowel syndrome will have a component of lactose intolerance. Later you may need to try a low FODMAP diet but it's difficult to stick on. Please reply if you want more details on low FODMAP diet. I would suggest a preparation of Ispagula husk, which contain soluble fibre and is effective diarrhoea and constipation to maintain stool consistency.
A probiotic preparation should also be taken. Please avoid self medication with over the counter drugs. If you don't get better with the above dietary modifications and drugs, you will need more evaluation and next line of treatment. Do reply for the same.
