Suggest Treatment For Chronic Sinus, Acid Reflux And Chronic Cough

I have had chronic sinus for nearly 1 year - after a sinus infection. I also have acid reflux which I take Somac for but have developed a chronic cough. Taking the Somac at night had reduced the night time cough, but I still cough up a lot of bile in the morning and sometimes vomit because of this. My GP wants to treat me for COPD even though Xrays and CT of my lungs and chest are clear but I believe my problem may be my gall bladder. Can you advise. I am seeing my oncologist for review in a month and considering a referral to investigate either my sinuses or gall bladder.

General Surgeon 's  Response
Hello,

As per my clinical experience, issue sounds secondary to chronic allergic condition with underlying GERD and secondary manifestations. My guidelines at present:
- Avoid triggers such as cold wind, dry air, animal dander, atmospheric pollen or industrial gases or chemical irritants
- Use of face mask (N95) is best way of prevention
- Deep breathing exercises and meditations help best
- Strict avoidance of active or passive smoking at any place
- Balanced nutritious diet to combat the immunity
- Acute cases do require bronchi dilator as short acting albuterol with low dose of inhalation corticosteroid with or without long acting beta agonist as Salmeterol or femoterol
- Chronic cases do response to montelukast, zafirlukast or zileuton as per the clinician concern
- Simultaneous control of acid reflux of GERD is best recommended with dietary measures, aerobics and proton pump inhibitors
- Control of blood sugar, cholesterol, BP, body weight

Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
