Suggest Treatment For Cold, Shivering And Weakness For A 68-year-old Female

Hi Doctor, at the very outset, please accept my regards. I am writing to you about my mother who is 68 years of age now. Since 2014 she has been a patient of pan-hypopituitarism, hypoglycemia, hyponatremia, hypokaelemia, hypothyroidism and DVT in her left limb. She was facing complications like frequent bouts of vomiting, hardly any appetite and seizures (not frequent though). She also used to feel cold in normal room temperature and finally very weak and both her legs bent and went stiff as a result of which she was unable to walk. Prior to 2014, she was fine and maintaned normal health condition as per her medical tests then. Later, in August 2015, she was diagnosed (MCDT Angiogram of Brain) with a giant intrasellar ICA aneurysm (1.9 cm X 1.3 cm) from the left internal carotid artery in her brain with a narrow neck extending into the sellar region. Major part of 2015 she spent in hospital with only intermittent returns to her home. When she got released from the last hospital in January 2016, doctors had prescribed her medicines like Wysolone 40 mg, Thyronorm 50 mg. Alongside these they also gave her Veloz-D, Calcimax Forte, Pan-40 and Alprax 0.5 mg. Doctors had categorically advised, in order to keep her alive, she needs to be kept on Wysolone as long as she lives and her blood pressure should be monitored and not allowed to increase. They also advised that the ICA aneurysm in her brain MAY BE causing compression on her pituitary gland which is the cause for all her ionic imbalance. After her release from the last hospital in January 2016, she was enjoying her meals and doing very well with the above mentioned medicines. After taking Wysolone 40 mg for 2 months till end of March 2016, doctor again routine checked her and tapered down her Wysolone to 20 mg keeping all other medicines same. From mid May, her blood pressure started rising and gradually scaled upto 160/120. This time, doctor gave her Telma 20 mg keeping all other above mentioned medicines as they were. So now it was Wysolone 20mg, Telma 20 mg, Thyronorm 50 mg and Veloz-Z, Calcimax Forte and Alprax 0.5mg. She was otherwise doing fine with no complication except for her high BP. Even after 2 weeks, since her BP was still on the higher side like 140/105, doctor again checked her and did some modification in her prescription. He tapered down Wysolone from 20 mg to 15 mg, Thyronorm from 50 mg to 25 mg, increased Telma from 20 mg to 40 mg keeping rest of the medicines same. Now, after 2 weeks her blood pressure dropped to 120/90. Her other blood reports like Sugar (Fasting), Sodium, Potassium are withing normal range, but T3=0.31, T4=4.30, TSH=0.23 and Haemoglobin dropped from 12.0 to 10.90. However now, we are observing, she has started developing, although in little measure, few of her earlier complications like feeling cold with little bit of shivering in room condition mostly in the night time between 9PM to 1AM, slight weakness and reduced appetite. In the above context, can you please advice if her complications, although of low measure, like feeling cold with some shivering, weakness and reduced appetite is because of tapering of Wysolone 20 mg to 15 mg or reducing power of Thyronorm from 50 mg to 25 mg? And if you can also suggest what can be done to avoid these complications? Please also let me know if there are medicines to counter the adverse side effects of steroids like Wysolone and how long can a human consume steroids?

The most frequent cause of cold, shivering and weakness at 68 years indicate community acquired pneumonia through strep or else. Symptomatic treatment with conventional antibiotics covering gram positive bacteria is essential and later on if required she may need a chest X-ray also.
