Thanks for your query and elucidate history.
The history of fever and medication noted.
The reports are showing anemia, increases WBC count suggesting an infection in a close-cavity like kidney, lungs or liver.
I would advise you to talk to your treating Doctor and advise Him/ Her the following tests.::
X-ray chest to rule out pneumonia or tuberculosis / pleural effusion.
Ultrasonography for abdomen for liver , kidneys and other organs ( always seen).
Blood culture and sensitivity: many times this helps a lot.
Urine : routine, microscopy , culture and sensitivity.
Other blood tests like:: urea, creatinine, sugar, liver function tests .
Malaria has to be tested at least for 2 more times.
There is a chance that you have co-existing - more than one problems and this is the reason that there is no relief in spite of so many antibiotics.

Please continue Monocef 2 gms daily for further 5 days.
I would give my patients additional Metronidazole and antimalarial treatment - what er call as a carpet treatment.

You can always opt for second opinion of another Doctor just in case the first one is missing on something.
The above tests may give clue for diagnosis and guideline for further treatment.
