Suggest Treatment For Fistula And Fissure Type Piles

I have fissure type piles and developed with fistula. I have this problem more than 10 years, I am afraid to go for surgery, I am not facing any serious problem and Im running my day to day work, What is your advise, can I go with this or should I go to consult doctor?
