Hello there,
I understand you concerns for your husband's health. His sugars are quite high. First of all whether insulin is required or can be controlled by medicines can be determined only after examining the patient thoroughly. Based on body weight BMI signs of insulin resistance. Also whether your husband has any symptoms of frequent urination, thirst and weight loss will help to decide whether or not insulin needs to be given. So a diabetologist needs to examine and determine if any blood tests are required (like kidney functions, liver functions) before starting therapy.
Diet wise changes need to be made but remember that whatever changes you make today need to be followed for a lifetime so bring changes gradually. try to cut down on all sugars, honey, jaggery. Use sugar free substitutes but sparingly. Reduce carbohydrate content of meals. Before giving a list of diet changes we need to know the patients weight, daily activity and current food habits. So please visit a dietician who can guide you through the same.
Ask your husband to include exercise in his daily routine (provided there is not heart related problem) which helps to bring down the sugars.
Hope this helps.
Take care.

Having to start him on anti diabetic drugs with jus fasting and pp levels in view is not a good idea. Since you don't know how long he has been havibg these high levels for and what changes it may have caused on his body. You need to visit a diabetologist for a through physical examination and with the following blood reports:

HbA1C, urea, creatinine

Urine spot microalbumin, urine R/M

It's necessary for the doctor to know if diabetes has started affecting his body systems already. Also, to put him on a medication, he needs to check the above reports.
