Suggest Treatment For Hydrocele

Hi there, I've had a hydrocele for about 40 yrs (I'm 62), it's been stable and not too much of a problem, but recently after it came under pressure when I was working on a car, it's swollen up to about the size of an orange, I've waited three days to see if it would reduce - it hasn't, but the discomfort has reduced. It is still more tender than before. The surgeon I saw previously said the safest operation is the one that isn't done, however I think that this larger size will be difficult for me to live with. Is there any method (activity, diet, abstinence) that I can undertake that may help the increased swelling reduce?

General Surgeon 's  Response
Hi.
Thanks for your query.
Noted your history of hydrocele for 40 years which has suddenly increased in size over the last 3 days, although the discomfort is reduced.
You want to know the method by which this can be reduced.
The most probable reasons for sudden increase in size is internal bleeding or infection, both of which cause a long term problem as the internal lining gets inflamed and can cause the size to increase as the reabsorption is going to be less than the secretions.
I would advise you the following:
First of all to undergo an ultrasonography with color doppler examination in addition to the clinical evaluation by a General Surgeon.
Tapping of the hydrocele under ultrasonography guidance can drain all the fluid, will know the character whether a hematoma or pus.
Second best chance is to undergo a formal operation so that recurrence is nullified.
A course of antibiotic if pus is there is required.
