I can understand your concern. From your history, cause of
hydrocephalus is not clear whether it is after trauma or it was present before but detected incidentally after
head injury. The
unconsciousness can be due to hydrocephalus or traumatic
brain injury. For reaching upto some diagnosis, you can provide information about his past history of headache, any
meningitis or cranial infection, complete CT report in another question.
Regarding treatment of hydrocephalus, it can be managed by ventricle-peritoneal shunt or endoscopic third
ventriculostomy (ETV). The decision of either treatment can be made by neurosurgeon based on type of hydrocephalus, CT finding of pre-pontine cistern and availability of endoscope.
In shunt, a tube is inserted which bypasses fluid of brain (CSF) into abdominal cavity. It thus relieves pressure in brain and thus leads to improvement in consciousness and mental function.
In ETV, a hard tube with inbuilt light and camera, known as endoscope is used to make a hole in fluid containing chamber (3rd ventricle). This hole provides new passage for
drainage of fluid and thus relieves pressure.
Both procedures have pros and cons. No one is perfect. Main problem with both surgery is the blockade of passage and thus recurrence.
One more point is normal CT scan in head injury patient who is unconscious, the usual cause is "diffuse axonal injury" if other causes are ruled out. I can not say whether it is there or not as some information is lacking. But you should ask your doctor about it as it is a type of serious injury.
