Suggest Treatment For Mucus Discharge From Rectum After Bowel Movement

I am 15 years old and weigh about 8 stone, i am having this problem of which a mucus substance comes out my rectum after a poo, i can poo normally but it mostly happen a few hours later, also this weird see through substance comes out my penis uncontrolablly after ejeculation and sometimes even when i dont ejaculate, with the bowel movements and this mucus, i am sometimes very gassy, please help
