We had gone to kolkata in May and consulted a psychiatrist there. He prescribed shicalm'H'- 1/2 tab in the morning and 1/2 tab in the a'noon and S-zelato plus- 1 tab at night for three weeks. After three weeks on review he prescribed etirest additionally to be taken 1/2 tab in the morning and 1/2 tab in the a'noon. The medicines have been continued for a month now since 24 May. For the last two days she is having several bouts of crying displaying lack of confidence and helplessness. What is to be done?

Psychiatrist 's  Response
hi, thanks for writing. i have understood your concerns,..

first of all some questions?
1. whether drugs have been stopped after 3 weeks of prescriptions?
2. whether she has shown any violent behavior or fearfulness, suspiciousness at the initial presentation?
3. whether there is any stressor associated with her crying for last two days?

she was given the appropriate drugs like szetalo plus, which alone can control the anxiety and depression. it is combination of escitalopram and clonazepam. clonazepam acts as a anxiolytic and also causes sleep.

I here wander why she was given shicalm h drug? it is antipsychotic drug combination. she does not need this. it may be given in the resistant cases but here she has received treatment for the first time.

if the drugs have been stopped after initial treatment then consult a psychiatrist and drugs needs to be started again. do not stop drug without such advice from a psychiatrist. it needs to be continued for t least 8 months after 1 st episode.

If she has worsened on medication then she needs to be evaluated again by a psychiatrist and drugs need to be changed..

i hope this advice will be helpful to you.. best of luck...
