Hi,
Thanks for contacting HCM.
According to your clinical history two probable differentials are paraphimosis and infection.
Paraphimosis is a medical condition in which foreskin of the penis become retracted and form a swollen ring of tissue around the penis.Long term catheterization is a common cause of paraphimosis especially in uncircumcised males.Any infection around penis also have the same clinical picture.
You can use hot compresses to reduce the pain and swelling and the patient definitely have some relief with that.Local application of lignocaine gel can also be tried.
But you must consult a urologist as soon as possible because paraphimosis is a urological emergency and early intervention favors good outcome.
Hope I have addressed your concern.
Thank you.
