My friend has recently had an ileostomy reversed but dshe is having problems with rectal burning due to the fact that so much of her intestines has been removed that her stools are still very acidic from stomach acid? What can she use to soothe this rectal burning?

Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Rectal burning after reversal of ileostomy is a well-known thing as the food does not get sufficient time for proper digestion and stays in the intestines; hence the stool will have all the components like pancreatic enzymes, bile and so on.

I would advise you the following in such a situation:
Send the stool for detailed lab tests.
Drink plenty of oral fluids.
Antacids when on empty stomach may help a bit.
Find the foods that help to reduce the symptoms.
There may be some foods and beverages that instigate or enhance the problems and hence should be avoided.

Apply Xylocaine jelly or ointment before passing stool.
Sitz bath in a tub with warm water.
Take a course of antibiotic, if there is any evidence of an intestinal infection.
Take care.
I hope I have answered your query. Let me know if you have any further questions.

Regards,
Dr. T Chandrakant
