Patient complains persistent tenderness right knee and groin Started few weeks ago after injury to right great toe, became red, swollen, warm, painful Toe improved without medical attention Knee/groin tenderness remain What could be the cause of the current compliant?

I have gone through your query and understand your concern. It may be due to lymph node enlargement due to injury to the great toe. Its treatment is analgesic such as ibuprofen for pain relief. It will gradually resolve. There may also be some ligament injury along with toe injury. Again rest and analgesic such as ibuprofen is the treatment.
