Suggest Treatment For Speech Impairment And Numbness On The Legs With History Of Major Stroke

My brother had a major stroke 10 years ago, caused from abuse of prescription drugs, (injecting hydrocodone) He has speech impairment and limited use of right leg and arm. He also has Hepatitis B and C. He also smokes a pack of cigarettes per day and drinks on a regular basis. I am his caregiver, he refuses to go to a doctor. What symptoms should I be watching for, what health problems might he be most susceptible to? What is possible prognosis for him?

here since it's already 10 years over after having stroke further improvement cannot be expected for speech as well as right hand and right leg. Here you also need to understand patient will have issues or problems after stroke as per the damage has happened in brain. Suppose the damage was not much then it must have been improved as of now for his speech as well as right hand and leg movement.
I suggest you to keep regular watch on his blood pressure as well suggest him to quit on smoking and drinking habits.
Major times i have seen the cause of stroke is either high blood pressure or clotting in blood vessels supplying blood to brain.
To answer your question what all symptoms you should be watching for your brother as a initial symptoms of stroke or alaraming are as below:
Loss of movement in hand and leg, difficulty in balance and coordination, too much of sweating on forehead, involunatory movement of leg or hand, too much of hedache and the alaram will be high blood pressure.
If your patient have any of this problem and can not be controlled at home then immediately take him to near by emergency hospital.
