Suggest Treatment For Throbbing In Left Shoulder/rib/neck

I have a good friend and he has had this throbbing, although not painful, it keeps him awake all the time. he can not lay on his left side or the throbbing increases, he states that it feels like a strong heartbeat. Sometimes it is in his left shoulder or under the left rib or left side of neck. he has been to physical therapy, chiropractor, and 15 different doctors. Has had Kenalog3. His job requires him to drive every day approx 3 to 4 hours a day. do you have any suggestions?
