What Cause Decrease Levothyroxine During Hypothyroidism?

I am a nurse practitioner and have a patient with hypothyroidism, recently decreased on Levothyroxine to 75 mcg/daily due to recent labs. TSH 0.034, thyroxine 9.5, T3 uptake 34.5, Free T4 3.3, T3 (thyronine) total 113, and anti-thyroglobulin 49. This pt. also has diabetes. He is c/o dry skin and hypothyroid type symptoms. ?autoimmune thyroiditis??? I work in a correctional facility.

You are case of hypothyroidism ..
So you are suspecting correct one that is autoimmune hashimoto thyroiditis ....

Along with antithyroglobulin , antimicrosomal ab will also be positive ....
FNAC can also be useful....

Let me know you , you might had not monitored your therapy by doc...
Here in report your TSH low and free t4 high ....
So dear you are developing hyperthyroid picture secondary to overcorrection of hypothyroid ...

So your dose can be reduced to 50 ug if it can maintain thyroid level within limit ...
Our aim is to maintain TSH betwwen 2 to 3 and accordingly adjust dose of levothyroxine ...

Consult your treating physician with keeping this in mind ..
