welcome to Health care magic.
1.What causes lump - Most possible cause in your age group can be a cyst followed by fibroadenoma, ductal ectasia.
2.The symptoms are periodic as you have said. This is possibly due to the ductal ectasia.
3.The pains are generally seen in the inflammatory cause.
4.However definitive diagnosis is done on the basis of the ultrasound scan ( which will help in characterising the lesion - cystic, solid, duct. And its extensions.
5.Possiblities of being cancer is rare, almost not seen in your age group.
Hope it helps you. Wish you a good health.
